If your child is enrolling in a Clark County school for the first time, or if you are re-enrolling your child in a Clark County school, you will need the following items to complete your child's registration:
Proof of address
One of the following proofs of address in your name is required to show you live in our zone: Power, Water or Gas. We will accept a fax from the utility company. (Driver's license, telephone bill, or cable bill is NOT acceptable.)
If you are living with a friend or relative, you must bring:
A notarized residential affidavit provided by the office signed by you and the owner/renter of the property where you are staying, indicating that it is your place of residence
The person you are living with and a form of your personal identification. We will keep a copy along with the residential affidavit
The residential affidavit must be completed each year
Proof of the child's identity
*Only needed if enrolling in CCSD for the first time.
This may consist of an original birth certificate, a passport, or a certified birth card (issued by the Clark County Health district or another health district). Copies will not be accepted.
All parents registering their child must provide a picture ID

Up-to-date medical information. You must have records indicating that your child has had this series of immunizations:
Five DPT Shots (One must be after the 4th birthday)
Four Polio Doses (One must be after the 4th birthday)
Two MMR Shots
Two Hepatitis A Shots
Three Hepatitis B Shots
Two Chicken Pox (Varicella) Shots

A link will be available in your Parent Portal Account at campus.ccsd.net/campus/portal. Make sure you are in the 2017-2018 school year.
New Families
New families to CCSD may complete online registration from home through the CCSD website at register.ccsd.net. Parents completing the registraton at home must have an active personal email account. If you would like assistance with completing registraton or do not have an active email account, please visit our school to complete online registration.
